It appears to be this after market front fender from V-Twin Manufacturing for a 16″ wheel on 41mm Wide Glide front forks.

Just lucky on my part. I've recently been purchasing fenders for my Street 750 chopper project from J&P Cycles and so I was fairly certain I'd seen it before. It would have been a lot harder to ID if it was a stock Harley fender.

Identifying currently selling after market parts is relatively easy but figuring out what some historical parts are can be very difficult. There are those that know them but that has to be tribal knowledge because they can't be researched easily.
